  • Regular progress reviews by the care team and individual check-ins

Individuals will select their preferences for Navigator communication.

Dia-B-Teams invites your existing providers to collaborate on one platform to help make recommendations and shared decisions for your overall health and diabetes management.

The Diabetes Navigator will also work with the team to identify gaps in your care network.
